NHS@Home Virtual Wards
We're supporting the Bath and North East Somerset, Swindon and Wiltshire (BSW) NHS@Home Virtual Wards service which gives suitable patients the same high level of care, assessment, monitoring and treatment from the comfort of their own homes, as they would get as an inpatient on a hospital ward.
NHS@Home Virtual Wards provides a range of interventions, tailored to meet the needs of the individual, to help prevent hospital admissions and also to accelerate discharge from hospital.
This service is also sometimes known as Hospital at Home.
How does NHS@Home Virtual Wards work?
NHS@Home Virtual Wards supports people at the place they call home, including care homes.
As a patient being looked after through the NHS@Home Virtual Ward service, you will stay in the comfort of your own home or place of care and will have an individual, personalised care plan designed to meet your specific care needs.
Support through NHS@Home Virtual Wards can sometimes include using monitoring technology via applications on a mobile phone and/or wearable technology. You will be given support on how to use any devices, and how our carers and staff will get in touch with you.
Our NHS@Home Virtual Wards team will schedule regular visits, dependent on individual need, to deliver the treatment and care required. You will also have access to advice and support outside of their visits.
Through regular monitoring, we will spot any signs of you becoming more unwell. If this happens, we will make sure you can access hospital care quickly and in a planned way.

At home, you’ll also be more likely to be comfortable, continue with daily routines, make choices about what you eat and wear, and have family and friends around you.
