How do I access my patient records?
You have the right to access the personal information Wiltshire Health and Care holds about you. You may ask to view your patient/health records in person, or you may ask for a copy of your health records.
In most circumstances the information will be provided to you free of charge. Wiltshire Health and Care may raise a charge for excessive, unfounded or repeated requests. In these circumstances we will inform you of the potential cost before responding to your request so that you can make a decision on whether you would like to proceed.
We will respond to your request within 1 calendar month of receiving it. Should additional time be required because of the complexity of the request we will write to let you know.
Please note, we will need to confirm your identity before disclosing information or releasing photocopies.
To request access to your patient records held by Wiltshire Health and Care:

- Phone 01249 456565 or email whc.recordsrequest@nhs.net and we can send you an application form.
How we use your personal information
