How do I access my patient records?
You have the right to access the personal information Wiltshire Health and Care holds about you. You may ask to view your patient/health records in person, or you may ask for a copy of your health records.
There is normally a charge for providing a copy; the amount depends on the quantity and size of photocopies/print-outs, and on whether any x-rays or medical photographs are to be provided on CD. The charge will not exceed £50.
To request access to your patient records held by Wiltshire Health and Care:
- Complete this application form and post it to the address on the form.
- Phone 01249 456565 or email firstname.lastname@example.org and we can send you an application form.
We should be able to provide you with access to your health records within 40 calendar days, although we aim to do so within 21 calendar days wherever possible. Please note, we will need to confirm your identity before disclosing information or releasing photocopies.
