Patient Records

How do I access my patient records?

You have the right to access the personal information Wiltshire Health and Care holds about you. You may ask to view your patient/health records in person, or you may ask for a copy of your health records.

In most circumstances the information will be provided to you free of charge.  Wiltshire Health and Care may raise a charge if there is an excessive amount of information in your record.  In these circumstances we will inform you of the potential cost before responding to your request so that you can make a decision on whether you would like to proceed.

We will respond to your request within 30 calendar days of receiving it.   Should additional time be required because of the complexity of the request we will write to let you know.

Please note, we will need to confirm your identity before disclosing information or releasing photocopies.

To request access to your patient records held by Wiltshire Health and Care:

We should be able to provide you with access to your health records within 40 calendar days, although we aim to do so within 21 calendar days wherever possible. Please note, we will need to confirm your identity before disclosing information or releasing photocopies.

How we use your personal information

Wiltshire Health and Care is registered with the Information Commissioner’s Office as a Data Controller, as required by the EU General Data Protection Regulations (GDPR) and UK Legislation.
